Error: Server USWVLIMS not found or server may be down (FWM 01003) null

Hi,

I ma unable to logon to CMC: it says the folwing error:
Error: Server USWVLIMS not found or server may be down (FWM 01003) null

and SIA agent is not working it is telling me to enter username and password and when i enter it is giving me a error
MYSQL DB: is working fine if i enter same user name and password:

unable to find CMS.exe in Task manager
